Moving with Medi-Cal: Inter-County Transfers

Homebase and Western Center on Law and Poverty offer this new Guide on Medi-Cal Inter-County Transfers, “Moving with Medi-Cal.”

Medi-Cal provides health coverage for people with low incomes throughout the state of California. While Medi-Cal is a statewide program, each of the 58 counties separately administers the program for most residents who live within that county. When someone moves to a new county and wants their Medi-Cal to continue, they need to ask for an “inter-county transfer.” The transfer process can be especially complicated for people experiencing homelessness.


Providers have expressed challenges in understanding how the inter-county transfer system is supposed to work. Many have shared stories that their efforts to assist clients have been unsuccessful.

 
The Guide and accompanying webinar aim to help advocates and providers, especially front-line staff, better understand these processes to help people keep their coverage during the inter-county transfer process.

 
The full Guide has detailed information about Medi-Cal’s inter-county transfer process. Within the Guide we’ve included sample stories about how people experiencing homelessness move through the inter-county transfer process.  In addition to the full Guide, we also offer two brief roadmaps (short and simple fact sheets) that outline the process for Part 1: Reporting a Move and Part 2: Enrolling in a New Medi-Cal Managed Care Plan.

Medicaid and Medi-Cal Renewals

People experiencing homelessness face unique barriers to renewing their Medicaid year after year. With a focus on California, Homebase compiled a report with lessons learned and recommendations for streamlining the renewals process for people who are unhoused.​

​After a 3-year pause, Medicaid is back to requiring members to renew their coverage every year. Millions of eligible members have lost their coverage since April 2023 due to missing their renewal deadline or never receiving renewal information in the first place. People experiencing homelessness are more at risk of losing coverage, as Medicaid members receive their renewal packets at their last known addresses. Individuals may not have a current mailing address or their address has changed in the past three years.
