The CalAIM Toolkit for Continuums of Care (CoCs) and CoC partners contains guidance and planning documents intended to support effective cross-system collaboration between homeless response system partners and local Medi-Cal managed care plans (MCPs) through the California Advancing and Innovating Medi-Cal (CalAIM) initiative.
The materials in this toolkit are intended to help CoCs communicate to MCPs about their own systems, goals, strengths, and limitations and engage with MCPs around housing and health coordination. This toolkit is designed to support communication and planning efforts between CoCs, CoC partners, and MCPs to reach the shared goal of improving the health and housing outcomes of people experiencing and at risk of homelessness.
The Toolkit can help build or strengthen partnerships. It is also meant to facilitate long-term cross-system coordination and collaboration to improve housing and health outcomes for those experiencing homelessness. Each document can be referenced and used separately, depending on the needs of each CoC and community.
